Multifocal Micropapillary Thyroid Cancers: Should We Adopt a More Conservative Approach?

Abstract Introduction There is a trend toward more conservative management of solitary micropapillary thyroid carcinomas (mPTC). Incidental findings multifocal mPTC are regular subject discussion at multidisciplinary team (MDT) meetings, however, guidance on its limited. Methods All patients discussed the West Scotland Thyroid Cancer MDT between November 2008 and July 2020 with (10mm or less) first surgical pathology specimen were included. Cases excluded, as any cases that would alter prognosis e.g. papillary carcinoma >10mm. Results 52 included in final analysis, representing 6% total cancer MDT. The incidence has increased over time. 76.9% (40) female mean age 48.6. Median follow up was 23.5 months (Range 1–119). 15.4% (8) had pathological lymphadenopathy pre-operative imaging. Capsular vascular invasion extrathyroid extension common (26.9%, 9.6% 32.7% respectively). 55.8% (29) underwent additional treatment after finding mPTC. Overall, 1 (1.9%) patient developed recurrence still alive 75 salvage treatment. died during up, not felt to be related their disease. Conclusion Multifocal an increasingly discussion. Recurrence rates low may respond well therapy. A approach appropriate strategy for this cohort.
